Somatic experiencing therapy is a body-based trauma treatment that works through physical sensation rather than through retelling the traumatic story. Developed by Dr. Peter Levine, it treats trauma as an incomplete physiological response held in the nervous system. Sessions focus on what the body is doing now, not on what happened then.

What Is the Core Idea Behind Somatic Experiencing?

The approach holds that trauma results from a survival response that never completed. When fight or flight is interrupted, the activated energy remains stored in the nervous system and continues driving symptoms long after the event.

Three symptom patterns follow from that incomplete response.

  • Hyperarousal: constant alertness, startle response, difficulty settling.
  • Hypoarousal: numbness, disconnection, flattened emotion.
  • Oscillation: rapid swings between the two states.

What Techniques Does It Use?

Somatic experiencing relies on 5 core techniques.

  1. Tracking, where the client notices physical sensation as it shifts.
  2. Titration, working with small pieces of activation rather than the whole memory.
  3. Pendulation, moving deliberately between activation and calm.
  4. Resourcing, drawing on internal or external sources of steadiness.
  5. Discharge, allowing the body to release held activation through shaking, temperature change, or breath.

Titration is the technique that distinguishes it most clearly from exposure-based models. The client never has to move through the full memory at full intensity.

What Does a Session Actually Look Like?

Sessions are conversational and slow. The therapist asks what the client notices in the body, waits, and follows the sensation rather than the story.

A client might report tightness in the chest. Rather than asking why, the therapist asks what happens next, whether the tightness has edges, and whether anything shifts as attention stays with it. The work moves at the pace the nervous system allows.

Do You Have to Talk About the Trauma?

No. Clients can work productively without describing events in detail, which is why the approach suits people who find narrative work overwhelming or who have no coherent memory of what happened.

Why Does This Matter for Addiction Recovery?

Substances often function as nervous system regulation. A person in chronic hyperarousal reaches for something sedating, and a person in shutdown reaches for something activating. Remove the substance without addressing the underlying dysregulation, and the drive to regulate remains.

How Does It Compare to EMDR?

Both are trauma treatments, and both work below the level of narrative. Three differences separate them.

  • Structure: EMDR follows an 8-phase protocol, somatic experiencing is less scripted.
  • Focus: EMDR targets specific memories, somatic experiencing targets present sensation.
  • Pacing: EMDR reprocesses within sessions, somatic experiencing titrates across them.
